Uncertainty Over Source Of Nigeria’s First Case As WHO Sounds Alarm Over ‘Complex’ Africa Spread, FG Quarantines High-Risk Contacts

WHO warns of an increasingly “complex” Ebola outbreak as the Bundibugyo strain spreads across countries in East and West Africa.

Source of Nigeria’s first infection remains uncertain as international health agencies investigate the index patient’s movements and possible exposure during travel through Uganda.

Federal Government quarantines high-risk contacts, activates emergency response measures and strengthens isolation and preparedness across hospitals and states.

Health Minister Ali Pate says the index patient is showing significant improvement, with no evidence so far of community transmission in Nigeria.

October 23, () – Uncertainty over the source of Nigeria’s first confirmed Ebola Virus Disease case has deepened as the World Health Organisation (WHO) warned that Africa is confronting an increasingly “complex” outbreak, with infections now recorded across countries in East and West Africa.

The warning came as the Federal Government intensified efforts to contain Nigeria’s confirmed case, quarantining high-risk contacts, isolating and monitoring others and strengthening preparedness across states and health facilities to prevent community transmission.

Nigeria confirmed its first case of the Bundibugyo strain on Tuesday, becoming the fourth African country affected by the current outbreak after the Democratic Republic of Congo (DRC), Uganda and Kenya.

WHO Regional Director for Africa, Mohamed Janabi, said during an online media briefing on Thursday that the widening geographical footprint of the virus had complicated containment efforts.

“We are facing a more complex outbreak landscape”, Janabi said.

Despite growing regional concern, the Federal Government has sought to allay fears of a wider outbreak in Nigeria, saying there is currently no evidence that the infection has spread into the community.

Coordinating Minister of Health and Social Welfare, Professor Muhammad Ali Pate, also disclosed that the index patient was showing significant signs of recovery and could make a full recovery.

Speaking at a briefing in Abuja, Pate said the patient was reportedly asymptomatic when she arrived in Nigeria but developed symptoms days later.

“I spoke with the clinicians today, and I understand that the laboratory results show significant improvement. There is a possibility that she will recover fully. However, we cannot take anything for granted.

“We currently have one confirmed case, and our priority is to contain it and prevent transmission into communities or a wider outbreak”, he said.

Uncertainty Over Source Of Infection

While Nigerian authorities have described the case as imported, precisely where the patient contracted the virus remains unresolved.

The index patient is a Ugandan-born woman married to a Swiss diplomat based in Abuja.

There have been conflicting accounts of her movements before arriving in Nigeria. Ugandan authorities had indicated that she travelled to eastern DRC before transiting through Uganda, but WHO and the Africa Centres for Disease Control and Prevention (Africa CDC) said they had received no validated information confirming that she travelled to the DRC.

A Swiss government statement said the woman was believed to have contracted the disease while travelling in Uganda.

Uganda has rejected suggestions that the infection originated within its territory, arguing that transit or travel through the country does not establish where the patient became infected.

WHO Emergency Response Manager, Patrick Otim, said health authorities were still working to reconstruct the woman’s travel history and determine the source of her infection.

The investigation has assumed greater significance after Africa CDC disclosed that Nigeria’s index patient and Kenya’s first Ebola patient both departed Entebbe on October 3.

Africa CDC Head of Emergency Preparedness and Response, Wessam Mankoula, said authorities were investigating whether there could have been an encounter between the two patients.

Africa CDC Director-General, Jean Kaseya, similarly said there was no validated information establishing that Nigeria’s index patient had travelled to the DRC.

WHO Representative in Nigeria, Pavel Ursu, said information made available to the organisation did not indicate travel outside Uganda before her journey to Nigeria.

Investigators are consequently attempting to establish whether the Nigerian infection was acquired in Uganda, connected to another infected traveller or resulted from another exposure that has yet to be identified.

Amid the investigation, the Federal Government has stepped up containment measures around the index case.

Pate said contact tracing had covered people at the hospital where the woman received treatment, her domestic contacts and individuals associated with a private medical facility she visited.

High-risk contacts have been placed in quarantine at a designated facility, while other identified contacts are being monitored in accordance with public health guidelines.

Nigeria had initially identified 44 contacts, although Africa CDC subsequently indicated that additional contacts had been found without immediately providing a revised figure.

The Nigeria Centre for Disease Control and Prevention (NCDC), in collaboration with the Federal Capital Territory Administration, has activated its incident management system to coordinate the response.

Authorities are also investigating health facilities visited by the patient between the onset of symptoms and her eventual hospitalisation and isolation.

Pate said federal hospitals had strengthened preparedness measures and urged state governments to ensure adequate bed capacity, functional isolation facilities and strict infection prevention and control procedures.

He also called for stronger cooperation between public health authorities and private medical practitioners, stressing the importance of quickly identifying and reporting suspected cases.

No Evidence Of Community Transmission

The Federal Government maintained that there was no evidence, so far, of community transmission and urged Nigerians not to panic.

Pate advised members of the public to wash their hands regularly and avoid direct contact with the blood or other body fluids of suspected or infected persons.

“Ebola is not an airborne disease. It is transmitted through direct contact with infected body fluids,” he said.

The government said it would continue to assess the situation and rely on scientific evidence in determining whether there was any need to alter plans for conferences, political campaigns or other large public gatherings.

It also warned against the circulation of unverified information on social media, urging people identified through contact tracing to cooperate with quarantine, isolation and monitoring directives.

Africa Battles Widening Outbreak

The Nigerian case comes amid growing concern over the spread of the Bundibugyo strain beyond the DRC, where the current outbreak began in May.

Kenya subsequently confirmed cases, while Uganda recorded infections earlier in the outbreak before being declared Ebola-free in August.

Kenyan authorities have intensified contact tracing following the country’s first infection, while Africa CDC and WHO continue to monitor cross-border transmission risks.

The DRC remains the epicentre of the outbreak, with more than 9,000 confirmed cases and over 4,000 deaths reported since May.

Africa CDC has warned that the situation represents an “extremely serious threat”, with the fatality rate in the DRC approaching 50 percent.

The geographical spread has increased concern about international transmission because of extensive movement between affected countries and major transport hubs.

Nigeria’s emergence as the first West African country affected by the current outbreak has further heightened concern, given its population and extensive domestic and international travel networks.

Background

Nigeria’s latest Ebola case is its first confirmed infection since the country successfully contained an outbreak in 2014.

That outbreak began when Liberian-American traveller Patrick Sawyer arrived in Lagos while infected with Ebola.

His arrival resulted in infections among healthcare workers and others who had contact with him, triggering an emergency response built around aggressive contact tracing, isolation, surveillance and infection-control measures.

Nigeria eventually contained the outbreak, and WHO declared the country Ebola-free in October 2014.

Pate said the experience gained from that episode had provided Nigeria with expertise and institutional capacity to respond to the latest threat. He urged Nigerians to remain calm, rely on information from recognised public health authorities and cooperate with measures aimed at preventing transmission.
